Look at the side of the paper. That came from a spiral notebook that someone took wasted time to make up. I find it hard to believe that an official document would be hand written in a notebook.

sorry to tell yall,but thats how the sheet actually looks that you sign in on,it aint nothing but a log-in note book…nothing official or non-official about it…..I’m married and the only official paperwork you get is the license itself….other than that ….it’s just a regular sign in sheet
